File a Public Health Complaint

If you have concerns about the condition of a restaurant, the food being served, or if you became ill after eating at a restaurant, please notify your local public health inspector office.  

Please note:

• Only submit your complaint once.
• Please provide as much detail as possible, i.e. your name and telephone number, the name and location of restaurant, the date and time of your visit, type of food or foods eaten or details of the unsanitary conditions witnessed.
• If you have become ill after eating at a restaurant, please include details of your symptoms and the date and time the symptoms started.
• Due to the volume, SHA public health inspectors will follow up on complaints by priority, based on the risk to the public.
