Chronic Disease Management
We provide education and supports to help people manage their health and have an independent and healthy life.
Health-care providers follow best practices and standards to help people improve their chronic health conditions and overall health. We are guided by the needs, goals, and experiences of the people we serve.
Our programs vary across Saskatchewan and are all supported by skilled professionals. Conditions supported include Diabetes, Kidney disease, Heart disease, Stroke, High Blood Pressure (Hypertension), Long Covid, Chronic Obstructive Pulmonary Disease (COPD) as well as, LiveWell with chronic conditions/LiveWell with Chronic Pain programming. Specialized programs that serve Saskatchewan but are based out of Saskatoon and/or Regina are Inflammatory Bowel Disease, Asthma, Cystic Fibrosis, Interstitial Lung Disease, Ears/Nose/Throat, Bleeding Disorders, Clinical Health Psychology, Adult Rheumatology and Pediatric Diabetes.
