Merger of Weyburn Primary Health Care Clinic and Weyburn Health Centre
The Saskatchewan Health Authority (SHA) actively seeks opportunities to better organize services and resources to deliver more reliable and consistent team‐based care as close to home as possible.
On October 4, 2021, the Weyburn Primary Health Care Clinic and Weyburn Health Centre will be co-locating at the current Weyburn Health Centre location. Primary health care services that were provided at these clinics will now be available at one location.
This initiative advances the Saskatchewan Health Authority’s Connected Care Strategy. Connected Care is the long-term transformational strategy of the SHA to ensure care is provided as close to home as possible by the right provider at the right time. It is our service delivery model.
“Merging these teams will allow physicians, nurse practitioners, and other health care professionals to more efficiently provide the best care to patients sooner and as close to home as possible and enhance team-based care in the community,” says Primary Health Care Director Trevor Tessier.
The co-located community-based facility will be located at the Weyburn Health Centre, 36 4th Street in Weyburn. Primary health care services offered at Weyburn Medical Clinic, located at 104-433 4th St. NE, remain unaffected.
