In order to guarantee full compliance of the Proof of Full COVID-19 Vaccination policy directive, remember to ensure your MyConnection account is active by Oct. 31, 2021.

  • MyConnection is used for submission of your proof of vaccination or enrolling in the monitored testing program.
    • Watch this Logging in to MyConnection video to learn how to create an account.
    • Note: MyConnection accounts are not yet available for physicians, volunteers, patient family partners, Knowledge Keepers or contracted individuals. Please be patient – communication will occur once this is in place.
  • MySaskHealthRecord will be used for obtaining your proof of vaccination documentation.
    • Team members who do not yet have an account are strongly encouraged to begin the registration process immediately to obtain proof of vaccination documentation.
