Intake Process
Why am I completing the Intake Form and Project Summary?
- Brings awareness to Clinical Standards and leadership of work being considered and rationale for why.
- Assists in reducing/preventing duplication of efforts.
- Highlights need to align across the province where possible.
- Flags groups to be aware of former health region documents and any other impacted processes.
- Assists Clinical Standards in assigning a coach, if needed.
An Intake Form is required for:
- Clinical Standards and Clinical Procedures
- Order Sets and Medical Directives
An Intake Form is recommended for:
- Clinical forms
- Patient Information and Education Resources (PIER)
- General Clinical Documents (work standards, learning modules, algorithms, etc.)

Project Overview
- The Project Overview is a living document that is updated/completed throughout the clinical document development process.
- ONLY PAGE 1 is completed at this stage and submitted with the Intake Form.

All new Clinical Standards Document Development topics will be placed on the Status Report (SMARTSHEET) by Clinical Standards.
Project LEAD will receive an automated update request email on the second Monday of every month to update their project details within the specified time.
