Clinical Pathways - Provider Information
A clinical pathway is a standardized approach for patients and providers to navigate and manage specified clinical conditions from prevention through to assessment and clinical management. A pathway includes the patient journey as well as the care providers' clinical processes that support the management of all aspects of the patient care plan.
Currently, there are many pathways that have been integrated into the Saskatchewan Health Authority (SHA) structure including both those developed and transitioned from Ministry of Health (MoH) as well as those initiated by the SHA. In an approach to create system efficiencies, our goal is to utilize an evidence-based approach and apply the provincial clinical pathway framework to standardize pathway development and reduce the variation in patient care. By reducing these variations, clinical pathways address disparities in access, quality, and outcomes of healthcare, improves the engagement of patient and family partners and the information that flows from the health system to patients and families.

Please note that effective March 1, 2022, the reorder and storage of printed materials that are designated within each clinical pathway have been transitioned from the Ministry of Health to the Saskatchewan Health Authority.
For replenishment of your inventory, we are asking users to reorder materials using the Clinical Pathway Provincial Print Shop Order Form (SHA 5002) below.
