In order to used resources more efficiently, consider provincial collaboration to develop clinical documents that benefit the entire SHA versus updating local documents.
Requesting a Change?
Complete the Clinical Practice Assessment Guide (CPAG) to request a change to content of a former health region clinical policy or procedure.
- Leadership must sign off/approve of the request PRIOR to submission.
- Submission of the form will initiate a practice consult to Clinical Standards & Professional Practice (CSPP)
- If approved, CSPP will facilitate the changes with the requestor.

Repeal Request
Use this form to request a repeal for a former health region clinical policy and/or procedure:
- outside of a project in which a repeals spreadsheet has been submitted to Clinical Standards; and/or
- discovered on Document Finder post-publication of new provincial clinical documents.
