What is Home Health Monitoring?
Home Health Monitoring (HHM) is the TELUS Health solution used within the Saskatchewan Health Authority for remote patient monitoring.
Remote patient monitoring (RPM) is a type of virtual care that uses digital technologies, like HHM, to collect health data from patients and electronically transmit that data to health-care providers for assessment and recommendations.

Requesting access
Contact the Virtual Care department to start the conversation.
Initial discussions will touch on vision and potential workflows with a demonstration of the features within the Home Health Monitoring solution. Development of the patient interview(s) and monitoring processes are done in collaboration with our department ensuring the technical and clinical components are in-sync.
Once access has been approved, you will receive an email invite with your login credentials.
Training resources for health providers
Basic training courses are available through MyConnection:
NOTE: Do not use Internet Explorer, as it is not supported.
- Go to MyConnection and log in.
- Click on "MyLearning", then click on "Access MyLearning".
- On the top banner bar of the webpage, click on "CLICK HERE TO SEARCH ALL LEARNING MODULES".
- In the search bar, type "Home Health Monitoring".
- Select the Home Health Monitoring course based on your permission level: Clinician, Admin Clerk, Care Team Member
- Click on "Enroll for Free".
