Who We Are

The Advance Care Planning Program (ACPP) is a provincial program providing support and services for health care professionals and the general public surrounding the topic of advance care planning. 

ACPP Health Educators serve the entire SHA and are available to answer questions regarding the process of advance care planning, Health Care Directives, Substitute Decision Makers, and Goals of Care as well as questions about the related procedures, processes, and documents being implemented across the SHA

Services Provided
  • Information, education, recommendations and coaching related to ACP, HCDs, Proxy, Substitute Decision Makers, Goals of Care and the Saskatchewan Medical Order for Scope of Treatment (SMOST)
  • Development of education, resources, and clinical tools
  • Information, clarification and interpretation of the ACP policies and procedures
  • Information on legislative and policy requirements as well as best practices
  • Consultations and/or referrals to supporting portfolios to ensure accurate information is provided
  • Support portfolios in the implementation of advance care planning processes
Services Not Provided
  • Performing case reviews or investigations
  • Involvement with discipline
  • Involvement with family meetings, direct contact with patients
