The Advance Care Planning Program (ACPP) is a provincial program providing support and services for health care professionals and the general public surrounding the topic of advance care planning.
ACPP Health Educators serve the entire SHA and are available to answer questions regarding the process of advance care planning, Health Care Directives, Substitute Decision Makers, and Goals of Care as well as questions about the related procedures, processes, and documents being implemented across the SHA
