Clinical Document Submission Form
- This form is submitted when the clinical document(s) are final and ready to be considered for approval.
- The completed Project Overview is submitted with this form.
- If submitting multiple documents that underwent the same development process with the same working group and stakeholders, only ONE submission form is required for the bundle of documents.

When submitting your clinical document, please ensure to send:
- submission form;
- project overview
- EDITABLE version on a clinical document template;
- working group/stakeholders list
- repeals list
- permissions for any copyrighted images or content used within the document
- implementation and communication plan

Project Overview
- The Project Overview is a living document that is updated/completed throughout the clinical document development process.
- Page 1 is completed and submitted with the Intake Form
- The completed Project Overview is submitted with the Submission Form when submitting the clinical document(s) for approval.

REMINDER
- Plan projects and "go live" dates appropriately as it can take up to 30 days or more for approval to final posting once submitted.
- Processes are in place when unforeseen events occur and urgent approval is required.
