Clinical forms
Provincial clinical forms are any forms, either paper or electronic, used by clinical staff.
- They may be used to document care provided. This documentation is customized to a specific patient it is used for and becomes a permanent part of the health record.
- Forms that are a permanent part of the health record must be approved by the interim Provincial Forms Committee.
- They may be used by clinical staff for clinical purposes, but not part of patient care, such as checklists, training/education records, etc.
- Clinical forms that are not a permanent part of the patient health record are reviewed and published by Clinical Standards.
