Audit Tools - Skin and Wound Care
Audit instructions
- Audit all wounds, with a maximum of 5 per unit/home
- Include all types of wounds (new, existing, surgical, pressure, etc.)
- Complete and submit form for each wound being audited
- Complete audit quarterly (any time within the quarter)
- Make note on chart (i.e.; write quarter audit) in order to avoid auditing same chart twice in same quarter
- Complete or assign corrective action as soon as possible following audit
- Refer to CS-CDCS-0029 Health Record Audits for guidance on performing audits

The paper worksheet below may be used but all data must be entered in the online form or you can directly enter the answers online.
- NOTE: The online form cannot be saved and returned to. All fields must be competed and submitted for each wound
