Convergence
Admission, Discharge and Transfer (ADT) events must be done on the shift that the event occurs. Information in the system must be current at all times; delays will result in inaccuracies and system flow concerns. Inputting of events and assessments is the responsibility of all trained users.
- Admitting a resident
Residents will be added into each home by designated provincial ADT users. Once residents are added, users in the home are required to admit resident into a bed. - Internal transfer
Moving a resident into an empty bed within the same home. This function is used for tasks such as changing room/bed or Program ID (type of care). - External transfer
Resident temporarily transferred out of home (return is anticipated). This function is used to identify when the resident in not in the home, and bed is held for that duration of time. Example: resident is admitted to acute care. - Bed swap
Used when two residents are trading rooms. - Resident discharge
When a resident passes away or moves out of the home. - Resident overview
The resident overview page is a snapshot of commonly needed information. It can also be used as a quick link to the page where the information is entered. - LTCF assessment
The LTCF Assessment must be completed on Day 4 following the residents' admission. An observation period occurs for 3 days prior to the LTCF Assessment being completed. An LTCF routine assessment is required every 3 months for the duration of the residents stay in the home. A significant change assessment is to be completed as needed for 'major change' in health status.
