APP Form and Who Needs to Fill it Out
Accountability and Performance Plan Form (APP)
The primary benefit of the APP is the structure that it creates for meaningful, two-way conversations between employees and supervisors.
The financial aspects connected to the APP are important to many, but not to all. For those at the top of their current pay range, or for those where the financial aspects aren’t a primary motivator, the APP means something different.
Regardless of whether you have room for financial gain through the APP process or not, the value for all participants is in the continuous feedback conversations that the form facilitates throughout the year.
Engaged leaders who regularly connect with team members about how they’re doing, what they’re working on, what successes and learning opportunities they’re experiencing, and what sort of support they might need (from their supervisor and/or from others) have a significant positive influence on the overall experience of coming to work each day.
This sort of highly engaged, actively present leadership contributes to a meaningful workplace experience where people feel like they belong and are safe to learn, contribute and challenge.
APP form

Effective March 1, 2024, the APP is exclusively a digital document. From this new website, employees can create, update and submit annual APPs for review.

As part of the 2023-2024 APP year-end process, all OOS employees are required to transfer their Word-based APP to the new digital format by March 31, 2024.
Who needs to fill out an APP?
Creating an APP at the start of a new fiscal year (April) is a meaningful exercise for all OOS employees.
All OOS employees are encouraged to create an APP at the start of each fiscal year. This applies to casual and part-time OOS employees, as well as those planning on retiring or going on a Leave of Absence before the end of the fiscal year.
OOS employees can work with their supervisor to determine what might be reasonable from a goal-setting perspective, taking their individual situation into account.
All OOS employees, regardless of start date, should fill out an APP. Start with one goal if you have to and then meet with your supervisor about your goal(s) and what sort of support you might need (from your supervisor and/or others) to achieve the results that matter most in the coming year.
