Chronic Obstructive Pulmonary Disease (COPD) Pathway – Provider Information
The Saskatchewan COPD Pathway aims to ensure that high quality, evidence-based care is delivered to people living with COPD in Saskatchewan. The COPD Pathway prioritizes early diagnosis and access to appropriate testing for people at risk for COPD as close to home as possible. Through guideline-directed care and access to interdisciplinary health-care providers, the COPD Pathway aims to support patients with self-management of their COPD, reduce emergency room visits and hospital admissions for COPD, and prevent disease progression.
Provincial Provider Pathway
Provincial Respiratory Referral Form
Respirologists, family doctors, and nurse practitioners can refer any person in the province using the Provincial Respiratory Referral Form. Additionally, people living with COPD have the option to self-refer for support services, education, tobacco cessation, pulmonary rehabilitation, and both clinic and home visits.
