Medication Reconciliation (MedRec) - Clinical Resources
Medication Reconciliation is a Required Organizational Practice (ROP) through Accreditation Canada for Medication Safety.
- MedRec is a systematic and collaborative review of all medications a patient has been taking and will need to continue,change or stop at each significant transition of care.
- Transition of care means the movement of patients along the continuum of care such as self or assisted-care, hospital admission, inpatient service change, inter-facility transfer, and move-in to long term care OR discharge to self or assisted-care.
- MedRec provides consistent tools for providers, patients, and families to evaluate each medication at each transition to improve patient safety and reduce medication discrepancies.

- There will be a staged implementation that will being in Regina.
- Implementation throughout the province will occur tentatively in Spring, 2025.
