Rebranding – Clinical Document Development
Saskatchewan Health Authority Clinical Standards team is taking the next steps in provincial standardization of clinical forms, moving away from former Health Region logo application.
This work will be done through a phase approach, with the first phase focused on replacement of former Health Region logos and identifiers with the appropriate SHA logo or identifier.

2. Review the space available on the form or document and determine rebranding application.
a. Form has both fRHA Logo and fRHA Name in Header or Footer
- Update header and footers with the appropriate geographical label.
- Replace fRHA logo with SHA Logo with Identifier - see below for logo and instructions to update with your chosen identifier.
- Ensure the clear zone to the nearest left/right margin is equal in height or width or the cross in the province symbol

- If there is no room for the SHA Logo with Identifier, then replace the logo with the SHA logo only.
- Ensure the logo is at least 1.5" wide by 0.39" high and has a clear zone around it that is equal to the height or width of the cross in the province symbol.
- If the SHA logo is already on the document and the former Health Region identified in the footer, these forms do not require rebranding.

b. Form only had room for a logo ONLY. There is not header or footer.
- Replace fRHA logo with SHA Logo with Identifier and follow instructions within to update with your chosen geographical identifier.
- Ensure the clear zone to the nearest left/right margin is equal to the height or width of the cross in the province symbol.
c. Form has no room for a logo
- Replace former health region name with appropriate geographic label.
3. Send rebranded form or document (editable version) to SHAProvincialForms@saskhealthauthority.ca to be added to the provincial repository. Please keep the source document for your records and processes as well.
- Send patient-facing forms by March 31 and all other forms by June 30 (both editable and PDF versions) to SHAProvincialForms@saskhealthauthority.ca so they can be added to the central repository.
Phase 2 will follow in the coming months, with SHA Clinical Standards reviewing documents in collaboration with document owners, and a consolidation plan will be developed to reduce the overall number of area specific forms to support provincial standardization where possible.
