- Copy of requisition MUST accompany specimen to SPH laboratory
- Fill in the ‘Other tests’ location on the Acute Care Phlebotomy Requisition Form # 101062 or the Community Lab Requisition Form #101064
- Mitogen Advanced Diagnostics Laboratory Out of Province Requisition Form; http://mitogen.ca/requisition-form/

Specimen types accepted
- Serum
Specimen collection container
- Preferred Collection Container: Gold (gel)
- Alternative Collection Container: Red (non-gel)
- Sterile tube or a polystyrene tube (CSF)
Required volume
- Optimal Volume: Full 5.0 mL collection tube
- Minimum Volume: 1.0 mL
(Submitting the minimum volume makes it impossible to repeat the test or perform confirmatory/reflex testing. In some situations, a minimum volume may require a second collection

- Specimens received where the collection protocols were not followed
