Description

- Carbapenemase Producing Organisms (CPOs) are a group of bacteria that possess genes enabling them to produce enzymes that deactivate all Beta lactam antibiotics including Carbapenems. One of the effective measures to prevent spread of CPOs in healthcare settings is the identification and isolation of colonized or infected patients as directed by Infection prevention and control.
Ordering Recommendations
- Symptomatic patients with travel history to affected areas as determined by Public Health/IPAC
- Screening for colonization in cases of suspected transmission to control spread of this organism in healthcare settings
- Patient lives with someone or has been a roommate in hospital with someone who is positive for CPO
Specimen Information
Specimen types accepted
- Rectal swab: insert into rectum 2-3 cm and rotate 5 times
- Tracheal aspirate
Required volume
Use liquid-based E-swab collection system (preferred) or M-40 Transystem swabs
- Sterile container also accepted for tracheal aspirates
Transport and stability
Transport at room temperature, as soon as possible. If delay anticipated, keep specimen at 4°C.

- 4°C stability 4 days

Relevant clinical history
- Patient received health care outside of Canada in the last 12 months
- Patient travel history to India, Sri Lanka, Bangladesh or Pakistan in the last 12 months

Methodology
- Chromogenic screening cultures are performed routinely followed by MALDI-TOF MS identification and confirmation of positive specimens
