Now available and due by Oct. 15: Self-declaration form for COVID-19 Proof of Full Vaccination
Effective Oct. 4, 2021, all Saskatchewan Health Authority (SHA) team members are asked to complete a self-declaration form regarding their vaccination status, intention to get vaccinated, intention to enroll in the SHA testing program at their cost, or intention to seek accommodation. This is an important step in the implementation of the COVID-19 Proof of Full Vaccination Policy Directive. The form is available online, and can be filled in from any desktop computer or mobile device. Please be patient if the site is busy and ensure that you complete your from prior to the deadline of Oct. 15, 2021.
