Patient’s Medical Homes: the future of primary health care
Patient’s Medical Homes are beginning to change health care in our province by focusing on care that meets patients' needs in their own communities. This team-based care model ensures that Saskatchewan residents have increased access to primary care, resulting in better patient outcomes and fostering healthier communities across the province.
In a Patient’s Medical Home, a team of health-care professionals like doctors, nurses, and social workers, work together as one team. They also work closely with other services in the health-care system and community. This allows health-care teams to better understand each patients’ unique needs and create personalized care plans for every stage of life.
The SHA is currently partnering with over 150 clinics, including fee-for-service, contracted, and SHA-operated facilities, to strengthen team-based care.
